What's Good in Tillsonburg? Oxford County Paramedics win Innovation Award for Remote Care Program

Strong patient outcomes helped secure the award.

Expertise in monitoring a person's health in their home has won Oxford County Paramedic Services an award. 

The Ontario Health System Quality & Innovation Award – recognizes how patients feel about the County's Remote Care Monitoring & Chronic Disease Management Program.

Superintendent of Community Paramedicine, Jamie Walter talks about how Oxford paramedics successfully track the health of a person with a chronic disease such as heart failure or diabetes.

The next step says Walter is to bring specialized equipment into the person's home to gather real-time health data that can be monitored remotely. 

The Remote Patient Care Monitoring and Chronic Disease Management program is paired with in-home visits by paramedics every approximately every 30 days for up to three months. But Walter says it's not a one-size-fits-all approach. 

Walter notes the Innovation award given to Oxford County Paramedic Services is based on the measured success of the program. 

Walter says the application process for the award included a patient testimonial from a survivor of 8 heart attacks and a stroke.  The patient said he felt supported, safer in his home and closer to a doctor who wasn't local. 

In a written statement, Ryan Hall, Chief of Oxford County Paramedic Services said his team is committed to community health and the public safety of the people who live in Oxford County.
